What You'll Be Doing:
Deliver Exceptional Care: Provide skilled nursing services based on individualized care plans, following physician orders.
Assess & Adapt: Conduct initial evaluations and ongoing assessments to adjust care plans as needed.
Empower Patients & Families: Educate patients and their families on disease management, medications, and home care techniques.
Coordinate Care: Work alongside physicians, therapists, and home health aides to ensure seamless patient support.
Stay Organized: Maintain accurate clinical documentation and ensure timely completion of OASIS assessments.
What We're Looking For:
Licensed RN: Active Registered Nurse license in the state where you'll be practicing.
Experienced & Compassionate: Minimum of 2 years of nursing experience (home health experience preferred).
Detail-Oriented & Independent: Ability to exercise clinical judgment and adapt care plans based on patient needs.
Team Player: Strong communication and collaboration skills to coordinate with healthcare professionals and support staff.
Tech-Savvy: Familiarity with ICD-10 coding and electronic medical records (EMR) is a plus!
